ÔÎÑÔÎËÈÏÈÄÛ È ÑÔÈÍÃÎËÈÏÈÄÛ ÏÐÈ ÍÅÊÎÒÎÐÛÕ ÇÀÁÎËÅÂÀÍÈßÕ (ËÈÏÈÄÎÇÛ)

Ðÿä çàáîëåâàíèé õàðàêòåðèçóåòñÿ íàêîïëåíèåì èçáûòî÷íûõ êîëè÷åñòâ äàííûõ ëèïèäîâ â êëåòêàõ, ÷àùå âñåãî â íåðâíûõ. Ýòè çàáîëåâàíèÿ ìîæíî ðàçäåëèòü íà 3 ãðóïïû: 1) áîëåçíè, îáóñëîâëåííûå

èñòèííîé äåìèåëèíèçàöèåé íåðâíûõ âîëîêîí, 2) ñôèíãîëèïèäîçû, 3) ëåéêîäèñòðîôèè.

Ïðè ðàññåÿííîì ñêëåðîçå, êîòîðûé îòíîñèòñÿ ê ïåðâîé ãðóïïå çàáîëåâàíèé, íàáëþäàåòñÿ óìåíüøåíèå ñîäåðæàíèÿ êàê ôîñôîëèïèäîâ (â ÷àñòíîñòè, ýòà - íîëàìèíïëàçìàëîãåíà), òàê è ñôèíãîëèïèäîâ â áåëîì âåùåñòâå ìîçãà; â ðåçóëüòàòå áåëîå âåùåñòâî ïî ñîñòàâó ñòàíîâèòñÿ ïîõîæèì íà ñåðîå.  áåëîì âåùåñòâå îáíàðóæèâàþòñÿ ýôèðû õîëåñòåðîëà, îòñóòñòâóþùèå â íîðìå, à ñïèííîìîçãîâàÿ æèäêîñòü õàðàêòåðèçóåòñÿ ïîâûøåííûì ñîäåðæàíèåì ôîñôîëèïèäîâ.

Ñôèíãîëèïèäîçàìè íàçûâàþò ãðóïïó íàñëåäñòâåííûõ çàáîëåâàíèé, ïðîÿâëÿþùèõñÿ ÷àùå âñåãî â äåòñêîì âîçðàñòå. Ýòè çàáîëåâàíèÿ îòíîñÿòñÿ ê áîëüøîé ãðóïïå ëèçîñîìíûõ áîëåçíåé (Neufeld, Lim, Shapiro, 1975).

Áîëåçíè íàêîïëåíèÿ ëèïèäîâ õàðàêòåðèçóþòñÿ ðÿäîì ïîñòîÿííûõ ïðèçíàêîâ: 1) â òêàíÿõ íàêàïëèâàþòñÿ ñëîæíûå ëèïèäû, ñòðóêòóðíûì êîìïîíåíòîì êîòîðûõ ÿâëÿåòñÿ öåðàìèä; 2) ñêîðîñòü ñèíòåçà çàïàñàåìîãî ëèïèäà ñðàâíèìà ñî ñêîðîñòüþ åãî áèîñèíòåçà ó çäîðîâûõ ëþäåé; 3) ïðè ýòèõ çàáîëåâàíèÿõ íàáëþäàåòñÿ íåäîñòàòîê ñïåöèôè÷íîãî ôåðìåíòà â ëèçîñîìàõ, íåîáõîäèìîãî äëÿ ãèäðîëèçà ëèïèäà; 4) ñòåïåíü ñíèæåíèÿ àêòèâíîñòè ôåðìåíòà âî âñåõ òêàíÿõ îäèíàêîâà. Ñ ó÷åòîì âñåõ âûøåèçëîæåííûõ ïðèçíàêîâ áûëè ðàçðàáîòàíû ñïåöèàëüíûå ìåòîäû äèàãíîñòèêè äàííûõ çàáîëåâàíèé. Ñòàëî âîçìîæíûì òàêæå âûÿâëÿòü ãåòåðîçèãîòíûõ íîñèòåëåé äåôåêòíûõ ãåíîâ, îòâåòñòâåííûõ çà ðàçâèòèå ýòèõ çàáîëåâàíèé, è îïðåäåëÿòü ñôèíãîëèïîäèñòðîôèþ ó ïëîäà. Äàííûå îá îñíîâíûõ ëèïèäîçàõ ïðèâåäåíû â òàáë. 25.1.

Ìíîæåñòâåííàÿ íåäîñòàòî÷íîñòü ñóëüôàòàç ïðèâîäèò ê íàêîïëåíèþ ñóëüôîãàëàêòîçèëöåðàìèäà, ñóëüôîñòåðîèäîâ è ïðîòåîãëèêàíîâ èç-çà îäíîâðåìåííîãî íåäîñòàòêà àöèëñóëüôàòàç À, Â, Ñ è ñòåðîèä-ñóëüôàòàçû.
